Testosterone replacement therapy has become a much bigger part of the men’s health conversation. More men are checking their testosterone, tracking symptoms, monitoring energy, and trying to make informed decisions about treatment.

That is a good thing.

But there is one issue that is still too often missed: fertility.

If you are using TRT, considering TRT, or trying to understand your hormone health, it is important to know this clearly: a testosterone blood test does not tell you whether your sperm health is normal.

You can know your total testosterone, free testosterone, SHBG, and other hormone markers, and still not know your sperm concentration, motility, or total motile sperm count. Those are different questions, and they require different tests.

TRT can affect sperm production

The reason this matters is biological.

Sperm production depends on signalling between the brain and the testes. Two key hormones involved are luteinising hormone, LH, and follicle-stimulating hormone, FSH. When testosterone is taken from outside the body, it can reduce that signalling.

The American Urological Association and American Society for Reproductive Medicine explain that exogenous testosterone can suppress gonadotropin secretion. Depending on the degree of suppression, sperm production may decrease or stop, potentially leading to azoospermia, meaning no sperm in the ejaculate. AUA/ASRM Guideline Part II

That does not mean every man on TRT is infertile. It also does not mean fertility cannot recover or be managed with medical support. But it does mean sperm testing should not be treated as optional if fertility matters to you now or in the future.

Feeling healthy does not prove sperm health

One of the tricky things about male fertility is that sperm problems often do not come with obvious symptoms.

You can have normal libido, normal erections, normal ejaculation, and still have low sperm count or poor motility. Mayo Clinic notes that male infertility can be caused by low sperm production, abnormal sperm function, or blockages, and that testosterone replacement therapy and anabolic steroid use can impair sperm production and decrease male fertility. That is why “I feel fine” is not the same as “my fertility is fine.”

And it is why a semen analysis, or a high-quality home sperm test as a first step, can be so useful.

What sperm testing actually tells you

A sperm test looks at the parameters that matter directly for male fertility. Depending on the test, this may include:

  • Sperm concentration
  • Motility
  • Progressive motility
  • Total motile sperm count

These markers are not shown on a testosterone blood test.

That is the key point: if fertility is part of the question, sperm testing belongs in the conversation.

Hormones and sperm health are connected, but they are not the same.

Hormone testing can help you understand testosterone-related biomarkers. Sperm testing helps you understand fertility potential more directly. Looking at both gives a more complete view than either one alone.
