I bought my first tub of creatine from a GNC in two thousand nineteen, and the guy behind the counter, a twenty-two-year-old with forearms the size of my calves, told me to take it with grape juice because “the sugar drives it into the muscle faster.” I went home, mixed five grams of creatine monohydrate into a glass of Welch’s, and drank it like it was medicine. Here’s the thing most people get wrong about creatine. They think it’s an energy supplement. But creatine is doing at least five different things inside your muscle tissue during a bulk, and the energy part might actually be the least interesting one.

That initial weight gain on the scale that scares everyone away is literally your muscles expanding with fluid at the cellular level.
And here’s where it gets wild. That cell swelling isn’t just cosmetic. It’s actually one of the triggers for muscle growth. When a muscle cell detects that its volume has increased, it interprets that swelling as an anabolic signal. The cell essentially reads the extra volume as a green light to start building. Some researchers have gone as far as suggesting that increased cell volumization may be the first step in muscle protein synthesis. Over weeks and months of training, those extra reps compound into significantly more total volume, and volume is the primary driver of hypertrophy. A twenty twenty-four meta-analysis published in Nutrients by Ziyu Wang and colleagues at Beijing Sport University analyzed twenty-three randomized controlled trials and found that creatine combined with resistance training increased upper body strength by an average of four point four three kilograms and lower body strength by eleven point three five kilograms compared to placebo. That’s not a marginal difference. That’s almost twenty-five pounds on your leg press that the placebo group didn’t get.
But here’s where creatine separates itself from every other supplement in the store, and I mean every single one. It doesn’t just help you perform better in the gym. It changes what happens inside the muscle fiber after you leave.
Your body produces a hormone called insulin-like growth factor one, or IGF-one, locally inside muscle tissue. IGF-one is one of the most potent anabolic signals your body makes naturally. It activates satellite cells, which are basically stem cells that live on the surface of your muscle fibers, waiting to be called into action. When IGF-one levels rise, these satellite cells wake up, start multiplying, and then fuse into existing muscle fibers, donating their nuclei. More nuclei means the muscle fiber can manage more territory, synthesize more protein, and ultimately grow larger. Research by Deldicque and colleagues published in two thousand five showed that creatine supplementation increased IGF-one expression in skeletal muscle. Not by a trivial amount. Enough that subsequent studies started investigating creatine’s role not just as a performance enhancer but as a genuine growth regulator.
And then there’s the other side of the equation. Your body also produces a protein called myostatin, and myostatin does the exact opposite of IGF-one. It’s a growth inhibitor. It’s the biological brake pedal on muscle development. Everyone has it. It’s the reason you can’t just eat protein indefinitely and turn into a superhero. I was eating about three hundred calories above maintenance, training four days a week on an upper-lower split, and my lifts had been stalling for about six weeks. One more thing that most creatine conversations completely skip over, and it might be the most fascinating part. And then there’s the question everyone asks and nobody gives a straight answer to. Does creatine work for everyone? The honest answer is almost everyone, but not identically. Your response to creatine depends partly on your baseline muscle creatine stores. A few early studies suggested a possible link to increased DHT, but subsequent research hasn’t consistently replicated that finding, and the International Society of Sports Nutrition’s position stand has stated there’s no strong evidence that creatine meaningfully alters testosterone or DHT levels in healthy adults. It doesn’t cause kidney damage in healthy people. This myth traces back to a single case report from nineteen ninety-eight involving a man who already had pre-existing kidney disease. Dozens of studies since then have found no adverse renal effects in healthy populations at recommended doses. It doesn’t cause hair loss, despite what the Reddit threads say.
