A New Approach to Shoulder Replacement: Faster Recovery, Less Invasion

A stemless, rotator cuff–preserving technique may transform how patients recover from osteoarthritis surgery.

Osteoarthritis is a widespread condition affecting patients’ joints through degeneration rather than infection. About 600 million people worldwide are struggling with this pain. The traditional treatment has been joint replacement surgery. However, this type of surgery causes significant trauma and requires a long recovery time. Kenneth Donohue from Yale University has presented a new method: a stemless, rotator cuff–intact anatomic total shoulder replacement, allowing patients to begin movement immediately after surgery.

What is a “shoulder”?

The commonly called “shoulder” is actually a joint formed by the humerus (arm), clavicle (collarbone), and scapula (shoulder blade). The uppermost part of the humerus acts as the “ball,” while the scapula forms a shallow “socket,” known as the glenoid. This “ball-and-socket” joint is surrounded by the rotator cuff, which consists of four muscles and tendons, along with other complex structures that allow multidirectional movement.

How does the original surgery work?

In traditional anatomic total shoulder replacement surgery, the main goal is to mimic the structure of a healthy shoulder. Surgeons typically need to cut or detach one of the rotator cuff tendons in order to place the implants. After the surgery, the tendon must be reattached, requiring the patient to keep the arm immobilized for a long period, which lengthens recovery time.

Another method is called reverse shoulder replacement, which is used for patients with severely damaged tendons. In this procedure, doctors reverse the natural structure of the joint. Surgeons place a metal ball where the socket normally is and attach a socket component to the shoulder blade, allowing other muscles to compensate for the damaged rotator cuff.

The new method

This new method is designed for patients whose rotator cuff tendons are still intact, so the healthy structures can remain undisturbed. “This new technology offers a less invasive way to perform an anatomic total shoulder replacement that does not require cutting or detaching the rotator cuff tendon,” said Donohue. “This is exciting because it offers the potential to significantly shorten recovery time. After rotator cuff–intact shoulder replacement, patients are allowed to begin moving the arm actively, such as reaching overhead, immediately following surgery.”

Previously, shoulder replacements required long “stems” inserted into the bone because implant materials could not easily attach securely. With advances in materials, surgeons can now remove only the damaged surface of the humerus and place a metal implant with microscopic pores that allow bone to grow into it. This eliminates the need for a long stem for stabilization. On the other side of the joint, the socket is smoothed and fitted with a plastic component. This reduces painful bone-to-bone friction, allowing patients to regain function more quickly after surgery.

Benefits

With this surgery, patients can begin moving their arm immediately after the procedure. Compared to traditional methods, they also require fewer follow-up hospital visits, since the procedure is less invasive to the bone and preserves more of the patient’s natural anatomy, leading to a healthier recovery overall.
