When people hear the word lithium, they usually think of rechargeable batteries that power electric vehicles, smartphones and laptops. Few realise that the same element has been quietly transforming another field for more than seven decades — not technology, but mental healthcare.
Long before lithium became synonymous with clean energy, it had already earned its place as one of psychiatry’s most effective medicines. Today, lithium carbonate remains the gold-standard treatment for bipolar disorder and is the only mood stabiliser with overwhelming evidence showing that it significantly reduces the risk of suicide. For countless patients and families, it has meant the difference between repeated psychiatric crises and a stable, productive life.
Lithium is a naturally occurring chemical element with atomic number three and belongs to the alkali metal group. It is the lightest metal known and derives its name from the Greek word lithos, meaning “stone.” While the world increasingly values lithium for its role in powering electric vehicles and renewable energy technologies, medicine has relied on it for decades to treat severe mood disorders.
In psychiatry, lithium is prescribed mainly as lithium carbonate, and less commonly as lithium citrate. It is primarily used to treat bipolar disorder, a condition marked by alternating episodes of mania and depression. It is also prescribed in selected patients with recurrent major depressive disorder, particularly when suicidal thoughts persist despite other treatments.
One of lithium’s greatest strengths lies in the way it acts on the brain’s chemistry. The human brain functions through billions of nerve cells that communicate using chemical messengers called neurotransmitters. In bipolar disorder, the delicate balance of these messengers is disturbed. Lithium helps restore this balance by regulating neurotransmitters such as serotonin, dopamine and noradrenaline, thereby reducing extreme mood fluctuations. It also influences intracellular signalling pathways, including glycogen synthase kinase-3 (GSK-3) and the inositol signalling system, mechanisms believed to enhance neuronal survival, improve synaptic plasticity and protect brain cells from damage. These complex actions explain why lithium not only stabilises mood but also provides long-term protection against recurrent episodes.
Perhaps lithium’s most remarkable achievement is its proven ability to reduce suicide. While many psychiatric medications improve symptoms, lithium consistently stands apart because it lowers the risk of suicidal behaviour and suicide deaths in people living with bipolar disorder. This unique anti-suicidal property has been demonstrated repeatedly in clinical research and remains one of the strongest reasons psychiatrists continue to prescribe it despite the availability of newer medications.
Despite these remarkable benefits, lithium treatment continues to carry a degree of stigma. One reason is its narrow therapeutic window, with therapeutic serum concentrations generally ranging between 0.5 and 1.2 mmol/L. Because the difference between an effective dose and a potentially toxic dose is relatively small, patients require regular blood tests to monitor lithium levels, kidney function and thyroid health. Moreover, adverse effects such as increased thirst, frequent urination, hand tremors, weight gain and changes in thyroid or kidney function may occur even at therapeutic doses, leading some patients to hesitate before starting treatment. However, psychiatrists emphasise that with careful monitoring, regular follow-up and appropriate dose adjustments, lithium remains one of the safest and most effective long-term treatments available for bipolar disorder.
Emerging research suggests that lithium’s benefits may extend well beyond mood disorders. Scientists are investigating whether low-dose lithium may protect the ageing brain, reduce oxidative stress and chronic inflammation, improve resilience against certain metabolic disorders and possibly delay the progression of neurodegenerative diseases such as Alzheimer’s disease. Although these findings remain under active investigation, they have generated considerable scientific interest in lithium’s broader role in human health.
Some researchers have also proposed that lithium may function as a beneficial trace element in human nutrition. Interestingly, regions with naturally higher lithium concentrations in drinking water and soil, including parts of northern Chile, tend to have greater environmental exposure to the element through water, crops and grains. Whether these naturally occurring trace amounts contribute to better mental health outcomes remains an important area of ongoing research.
India’s own relationship with lithium entered a new chapter following the discovery of an estimated 5.9 million tonnes of inferred lithium resources in Reasi district of Jammu and Kashmir. While this discovery has attracted attention because of its importance for battery manufacturing and India’s clean-energy ambitions, it also highlights another, often overlooked, value of this remarkable element — its ability to save lives through medicine.
As psychiatrist Leonardo Baldaçara of the Universidade Federal do Tocantins, Brazil, has observed, lithium carbonate plays an essential role in the management of mood disorders because of its robust mood-stabilising efficacy and its well-established protective effect against suicide. His observation reflects what decades of psychiatric research have consistently demonstrated — that lithium remains one of the most valuable medicines in modern mental healthcare.
Mental illness continues to affect millions of people worldwide, yet stigma surrounding psychiatric treatment often prevents patients from seeking timely care. Lithium carbonate reminds us that some of the most powerful medical breakthroughs are not necessarily the newest ones. For thousands of individuals living with bipolar disorder, this simple medicine has restored emotional stability, prevented relapse and, most importantly, saved lives.
As the world celebrates lithium as the fuel of the clean-energy revolution, it is equally important to recognise another legacy of this extraordinary element — its quiet but enduring contribution to protecting the human mind and preserving life.
