One of the most common questions I hear from new patients has nothing to do with neck pain, back pain or chiropractic care itself. It’s about insurance. More specifically, people want to know if chiropractic visits are covered and how reimbursement actually works in Ontario.

The short answer on OHIP
OHIP does not cover chiropractic care for most Ontarians. This surprises some people, since OHIP covers a wide range of other healthcare services, but chiropractic care has been outside OHIP coverage for adults for a number of years now. This is a provincial policy decision, not something specific to any individual clinic or practitioner.

What extended health benefits typically cover
Most people who do have chiropractic coverage get it through an extended health benefits plan, usually provided through an employer, though some people purchase private extended health coverage independently. Some have an annual dollar maximum for chiropractic care, others cover a set number of visits per year and the percentage of each visit’s cost that’s reimbursed also varies by provider and plan tier.
Because of this variation, I’d encourage anyone considering chiropractic care to check their specific plan details before their first visit, rather than assuming a certain level of coverage. Most insurance providers have this information available through an online member portal or you can call them directly and ask specifically about chiropractic coverage, annual maximums and reimbursement percentage.
How the receipt and claims process actually works
At Annex Family Chiropractic, an official receipt is issued after every visit, containing all the information most insurers require for a claim submission: the practitioner’s registration number, the date and nature of the service and the amount paid. Patients typically pay for the visit directly and then submit that receipt to their insurance provider for reimbursement, though the exact process depends on your specific insurer and plan.
Some insurance plans also offer direct billing, where the clinic submits the claim on your behalf and you’re only responsible for any portion not covered by your plan. Availability of direct billing varies by insurer, so it’s worth asking your provider’s office whether this is an option for your specific plan.
What to actually ask your insurer
To get the clearest answers from your insurance provider, ask these exact questions:
- What is my annual maximum for chiropractic care specifically? (Is it bundled with physio/massage or listed separately?)
- What percentage of each visit is reimbursed? (Does it cover 100% up to the limit or a lower copay per visit?)
- Is a doctor’s referral required for reimbursement? (Even though chiropractors are primary contact providers in Ontario, some stricter plans still ask for this.)
- Does my plan reset on the calendar year or on a specific policy anniversary date? (This affects how you might want to time visits near the end of a coverage period.)

This is a genuinely personal decision that depends on your specific situation, your budget and what you’re hoping to address. I’d encourage anyone weighing this decision to have an honest conversation with a prospective provider about what an assessment and potential care plan would actually cost and what realistic outcomes might look like for their specific concern, before deciding whether it’s the right investment for them.
Do I need a doctor’s referral to see a chiropractor in Ontario?
No. Chiropractors in Ontario are regulated primary contact health professionals, meaning patients can book directly without a physician referral. Some insurance plans may still request documentation for reimbursement purposes, but that’s a plan-specific requirement rather than a general rule for accessing chiropractic care.
A note on provider directories
Annex Family Chiropractic is listed on Lumino Health, Sun Life’s healthcare provider directory, which some patients use to help confirm coverage details or locate a provider recognized by their plan. Being listed on a provider directory isn’t the same thing as a guarantee of coverage, since that still depends entirely on your individual plan, but it can be a useful reference point when you’re trying to sort out your specific benefits.
Why this conversation matters to me
I bring this up directly with new patients because I think surprises about cost are one of the more avoidable sources of frustration in healthcare generally. Nobody should find out how their coverage actually works only after they’ve already committed to a care plan. If a patient’s coverage is limited or they don’t have coverage at all, that’s useful information for us to know upfront too, since it can reasonably factor into how a care plan is structured and paced.
A few minutes spent checking your benefits before your first appointment can prevent unnecessary surprises later and help you make informed decisions about your care.
