Most people assume the hardest part of ICL surgery is correcting the prescription.
But actually, long before surgery even begins, there’s another question your surgeon has to answer first:
What size lens actually fits inside your eye?
And surprisingly, that matters a lot more than most people realise.
When you hear the word “lens,” what do you think of? Most imagine something like glasses or contact lenses.
I’m sure you know that contact lenses aren’t a one-size-fits-all.
They have:
- different powers
- different curves
- different diameters
Some feel comfortable immediately. Others feel dry, unstable, or irritating.
ICLs work on the same basic principle: the lens must match the anatomy of your eye. But there’s one major difference.
Unlike glasses and contact lenses, you can’t simply “try them on.”
Instead, we rely on highly detailed imaging technology to predict the ideal size before surgery ever takes place.
Unlike LASIK, which reshapes the outer part of your eye (the cornea) using laser treatment, an ICL is placed inside the eye.
Specifically, it sits in the small space between your iris and your natural lens. This space is called the vault.
That space is surprisingly delicate. Too much pressure in either direction can potentially cause problems over time.
So during your screening appointment, the anatomy of the front part of your eye must be carefully measured using high-resolution scans.
These measurements help estimate:
- How much room is available
- How the lens will sit
- How fluid will circulate around it
- And how much clearance will remain between the ICL and your natural lens
From there, your surgeon selects the most appropriate size from the available lens options.
When sizing is correct, the lens essentially becomes invisible to your awareness.
You don’t feel it. You don’t notice it. It simply becomes part of the optical system of your eye.
If the ICL is slightly too large for your eye, it may sit a little too high once inside. This creates a larger gap between the ICL and your natural lens — something we call a higher vault.
When that gap becomes excessively high, it can sometimes affect how fluid circulates inside the eye or place pressure on nearby structures, which may increase eye pressure.
On the other hand, if the ICL is slightly too small for your eye, it may sit too close to your natural lens, creating a lower vault.
If left unchecked over many years, that close positioning could theoretically increase the risk of cataract formation.
The important thing to note is that these changes do not suddenly appear overnight.
They develop gradually and are very easy for surgeons to monitor during routine follow-up appointments.
At your postoperative visits, your surgeon checks:
- How the lens is sitting
- the vault height
- your eye pressure
- and the overall health of the eye
So even in the uncommon event that the sizing isn’t right, it’s usually recognised early and handled appropriately.
Technology has changed ICL safety dramatically over the past few years. ICL sizing today is far more sophisticated than it used to be.
Modern imaging systems allow surgeons to measure the eye on an incredibly fine scale, greatly improving the predictability of sizing across many different eye shapes and prescriptions.
Even when postoperative measurements aren’t identical to the original prediction, the eye often naturally settles as healing progresses.
The body and lens frequently find a stable balance on their own.
When the sizing is correct — which it almost always is — patients typically stop thinking about the lens entirely.
That’s actually one of the most fascinating parts of ICL surgery.
The lens is a foreign object, and yet your body accepts it with barely a hiccup.
Vision simply becomes clearer, more stable, and less dependent on glasses or contact lenses.
And that’s really the goal.
Not just better eyesight. But visual freedom.
