When most people hear about GLP-1 medications, they immediately associate them with type 2 diabetes or weight loss. That narrative has become so dominant that it often overlooks a growing conversation: how these medications may benefit individuals living with type 1 diabetes.

As someone navigating type 1 diabetes, I’ve seen firsthand that blood sugar management isn’t just about insulin. it’s about timing, variability, hormone response, and the constant balancing act between highs and lows. While insulin remains essentials, it doesn’t address every piece of the puzzle. This is where GLP-1 receptor agonists may offer additional support.

What GLP-1 Medications Actually Do

GLP-1 receptor agonists mimic an incretin hormone involved in glucose regulation. Their effects include:

  • Glucose-dependent insulin secretion
  • Suppression of glucagon
  • Slowed gastric emptying
  • Increased satiety

These mechanisms help reduce postprandial glucose excursions and glycemic variability — factors that are particularly difficult to control in type 1 diabetes.

Why This Feels Different Than Insulin Alone

Insulin therapy replaces insulin deficiency. However, insulin does not address glucagon dysregulation, gastric emptying, and appetite signaling. This is where GLP-1s target those gaps that insulin does not. Clinically, this translates to smoother post-meal glucose control, lower insulin doses, and reduced glycemic variability.

Side Effects: The Reality

The most common barrier to GLP-1 use is GI discomfort. Many report nausea, vomiting, diarrhea or constipation. These effects are linked to delayed gastric emptying and are most prominent at initiation and during dose escalation.

Combatting Nausea

  1. Dose Titration: Start low, increase gradually, one of the most important strategies for tolerance.
  2. Meal Structure: Small, frequent meals and avoiding high-fat, large meals (worsen delayed gastric emptying).
  3. Eating Behaviors: Eat slowly, stop before fullness, and avoid lying down after meals.
  4. Hydration: Sip fluids throughout the day and avoid large fluid intake with meals.
  5. Time Course: Symptoms often improve within weeks as the body adapts.

A Shift in the Conversation

The traditional divide between type 1 and type 2 diabetes treatment is becoming less rigid. Although, GLP-1s are not FDA-approved for T1D and are not guideline-recommended (yet). There is increasing off-label use, growing clinical interest, and ongoing research. Even recent guideline discussions suggest that a role for adjunct therapies like GLP-1s may be emerging, particularly for metabolic and cardiovascular health.

I see the growing need for GLP-1s to make a crossover every single day, with my personal use of a GLP-1, I’m able to have tighter control of my blood sugars and reduce the amount of insulin I take through my insulin pump by nearly 40%. Maybe the question isn’t whether GLP-1s “belong” in type 1 diabetes care — but whether we’re ready to rethink the boundaries of how we treat it and the benefits that are presented through a medication like this.
