Let me ask you something.
When was the last time your doctor tested your grip strength?
Or whether you can stand on one leg for ten seconds?
Or get up off the floor without using your hands?
Probably never. And yet those three things — along with two others — predict how long you live, and how well you live, with more accuracy than most of the bloodwork sitting in your patient file right now.
I’ve spent nearly thirty years coaching people on fat loss, strength, and health. And the older I get, the more I realize we’ve been asking the wrong question.
The fitness industry is obsessed with how people look. Science is obsessed with how long they live.
Those are two very different conversations. If you’re over 40, the second one deserves your attention.
What Your Doctor Isn’t Testing
The data on what actually predicts mortality after 60 isn’t buried in some obscure database. It’s published in JAMA, the British Journal of Sports Medicine, and the European Journal of Preventive Cardiology. Some of the most respected medical journals in the world. We’re talking tens of millions of people studied over years and decades.
And the same picture keeps coming up.
The best predictors of whether you’ll be active and independent in your 60s, 70s, and 80s aren’t your cholesterol levels or your fasting glucose. They’re physical. Measurable. And you can test every single one in your living room, right now, for free, in under ten minutes.
Here are the five.
Test 1: Grip Strength
What it predicts: Your heart. Your survival. Your whole-body muscle reserve.
A 2017 study in JAMDA analyzed data from over 3 million participants and found that people with the lowest grip strength had a 41% higher all-cause mortality rate than those with the strongest grip strength.
Forty-one percent. That’s not a small finding. That’s a siren going off.
Grip strength isn’t just about your hands. It’s a window into your total muscle mass, your cardiovascular resilience, and your inflammation levels. When grip weakens, it’s a signal that other things are weakening too — quietly, before any symptom shows up on a scan or in a blood panel.
The test: The clinical gold standard is a hand-dynamometer squeeze test. For women, a weight below 20 kg indicates an elevated risk, and for men, a weight below 30 kg indicates an elevated risk. Most physio clinics can test you in under two minutes. At home, two options work well.
Dead hang from a bar — under 15 seconds is a red flag for both men and women. Alternatively, grab two dumbbells totaling roughly half your body weight — so a 160 lb person would use two 40 lb dumbbells — hold them at your sides and see how long you can stand. Under 30 seconds is worth addressing, 30 to 60 seconds is moderate, and 60 seconds or more is solid.
Not there yet? Start lighter and work up. If opening a jar, carrying groceries, or holding a full kettle has quietly become harder over the years, your body is already telling you something.
The fix isn’t complicated. Lift weights.
Compound movements that challenge your grip and your whole body — deadlifts, rows, farmer carries. People who build that habit in their 40s and 50s are creating something that pays off for decades.

Test 2: One-Leg Balance
What it predicts: Your 10-year mortality risk.
Stand on one leg. No wall. No hands. Can you hold it for ten seconds?
A 2022 study in the British Journal of Sports Medicine followed adults aged 51 to 75. Those who couldn’t hold a 10-second one-leg stand had 84% higher mortality over the following decade — even after accounting for age, sex, and existing health conditions.
Let that sit for a second.
Balance draws on three systems at once — your vestibular system, your vision, and your neuromuscular system. All three start declining in your 50s. Silently. And by the time most people notice it, the decline has already been happening for years.
A fall at 70 isn’t just a bad day. It starts a chain reaction — hip fracture, surgery, immobility, muscle loss — that the body never fully comes back from.
But here’s the thing. Balance is trainable. Single-leg exercises, stability work, and even standing on one foot while you brush your teeth start rebuilding those systems.
Can you hold ten seconds right now? That answer tells you something your doctor probably hasn’t asked.
Test 3: Daily Step Count
What it predicts: How long you live — starting today.
A 2022 meta-analysis published in The Lancet Public Health pooled data from 15 international cohorts and nearly 50,000 adults. The finding was straightforward: people who walked more steps each day had a 40–53% lower risk of premature death compared to the least active group.
Not gym sessions. Not supplements. Steps.
The sweet spot wasn’t 10,000 either — the mortality benefit leveled off at around 7,000–9,000 steps per day. Which means most people are leaving significant protection on the table, not because they can’t get there, but because they’ve never thought of their daily movement as a life-or-death variable.
Here’s what makes this one so important for the busy parents and professionals I coach. Your formal workouts — three or four sessions a week — account for maybe 5% of your waking hours. What you do with the other 95% is called NEAT: non-exercise activity thermogenesis. It’s your steps, your errands, your walks, your standing versus sitting. And it is a far bigger driver of both calorie burn and long-term mortality than most people realize.
A person walking 4,000 steps a day and a person walking 9,000 steps a day can be doing the exact same workouts and following the same diet. Their health trajectories are not the same.
The fix is the most accessible one on this entire list. Walk more. Every day. Make it non-negotiable.

Test 4: Sit-to-Rise Score
What it predicts: How long you live.
Sit cross-legged on the floor. Now stand up. No hands touching the ground, no knee dropping, no grabbing a chair. Every time you need support, you lose a point from a score of ten.
A 2014 study in the European Journal of Preventive Cardiology followed over 2,000 adults for an average of 6.3 years. Every single one-point improvement in the sit-to-rise score corresponded to a 21% improvement in survival.
Most people try this test and are genuinely surprised by how hard it is. That’s the point.
Getting off the floor without support requires strength, flexibility, balance, and coordination firing together. It’s a full-body audit in one movement. If you struggle, your body is giving you information worth listening to.
The good news is this one responds fast. Mobility work, leg strengthening, and simply practicing the movement regularly can shift your score in weeks, not years.

Test 5: Cardiorespiratory Fitness
What it predicts: Just about everything.
VO2 max — your body’s capacity to use oxygen during exercise — drops roughly 10% per decade after 30 if you don’t do something about it. A 2024 meta-analysis in the British Journal of Sports Medicine, drawing on 20.9 million observations, found that high cardiorespiratory fitness was associated with 53% lower all-cause mortality compared to low fitness.
Sit with that number for a moment. 53% lower all-cause mortality. Across 20.9 million observations. No drug on the market comes close. And you can do something about it starting this week.
Most people get this part wrong. Zone 2 cardio — the easy, conversational pace — builds your aerobic base and is genuinely good for your health. But if you want to actually raise your VO2 max, you need to work harder than that.
That doesn’t require a gym or expensive equipment. Burpees, jumping jacks, mountain climbers — calisthenics done in short hard bursts with brief rest periods challenge your cardiovascular system just as effectively as a spin bike or a treadmill. A 20-minute bodyweight circuit where you’re genuinely breathless by the end is doing exactly what needs to be done. Cycling hard, hill sprints, swimming with effort, or any interval-based class where you’re pushing into uncomfortable territory for 20 to 40 seconds at a time — all of it counts.
The rule is simple. Comfortable cardio maintains fitness. Hard cardio builds it. Two to three sessions a week, where you’re actually working — not just moving — are what shift that number over time.
You don’t need to train like an athlete. But you do need to stress the system.

What All of This Actually Means
Most people measure what’s easy to measure. The number on the scale. The cholesterol result. The blood pressure reading at the doctor’s office. But the body doesn’t break where the tests are. It breaks where no one ever looked.
Whether 80 years old means a walk on the beach or a wheelchair isn’t decided in a hospital. It’s decided in your 40s and 50s, in the small choices that feel optional right now because nothing has gone wrong yet.
I’ve seen this play out hundreds of times over three decades of coaching. The people who are genuinely strong and capable at 65, 70, 75 — they didn’t get lucky with genetics. They started building their body before they had to.
Grip. Balance. Steps. Floor mobility. Cardiovascular fitness.
Five tests. Free. Doable today. Backed by more evidence than most things your doctor will mention at your next physical.
Take them. Then train accordingly.
Because the goal was never just to look good in one photo.
The goal is to be strong enough to actually live your life and take many.
