Constanze: What if your skin could get a lift without surgery? Today at Amazing As You Are, we’re delving into the world of the Non-Surgical Facelift, or NSF. And that’s a very interesting topic. Joining us today is Dr. Marguérite van Randwijck. She’s a KNMG cosmetic physician and lifestyle doctor, as well as the founder of AAYA.

Dr. Maxime Hoogveld, great to have you here. And who else is on the show today, Marguérite?

Marguérite: Today’s show features…

Maxime: Renske Ypma.

Marguérite: Renske Ypma. She’s a skin therapist at Renske Ypma Skin Clinics.

Constanze: And you work together in Zandvoort, too, right?

Marguérite: We work together in Zandvoort, Amsterdam…

Constanze: Great.

Also on the show is Susan Hoffman, who’ll be asking viewers questions. And soon we’ll continue with Dr. Maxime and Dr. Marguérite in our doctor’s consultation.

AAYA TV — DOCTOR’S CONSULT

Well, there’s some very good news because… There is a non-surgical facelift option, and that’s the Non-Surgical Facelift. We have the Doctor’s Consultation with Dr. Marguérite and Dr. Maxime, and you can tell us a little more about it. What does that Non-Surgical Facelift actually mean?

Marguérite: Shall I begin?

Maxime: That’s good.

Marguérite: Well, we both do it, right?

And the great thing is, for people who don’t like incisions because they find them really scary, there’s an alternative to ensure that the skin that loosens with age is firmer. And you can do that with various techniques. And that combination of techniques — and we’ve been using that for a while now, haven’t we, Maxime? — it ensures that you can simply go back a few years without surgery.

And that’s essentially what a facelift does.

Constanze: And what kind of results can you expect? Because with a facelift, you know, you get firmer.

What happens with a Non-Surgical Facelift?

Maxime: Well, it’s not a matter of firm, firmer, firmest with a Non-Surgical Facelift, but I don’t think that should be the case with a surgical one either.

Maxime Hoogveld, Cosmetic Docter at AAYA

Maxime Hoogveld, Cosmetic Docter at AAYA

Constanze: Because you look strange, those wrinkles on your neck, and then this…

Maxime: Yes, which is, of course, what we all see on TV — what we don’t want, which we all say, well, that’s not how we want to look.

So it’s mainly a matter of looking at what suits you, your face, and your age.

Marguérite: Sometimes, I think surgery is the best solution for someone. Sometimes the skin is really sagging. And then it’s quite difficult to use devices and injectables to make that skin fresher and firmer again.

Because that’s what we’re actually doing.

Those skin cells start working less and less as you get older, starting around age 25. And you have to activate those cells. I think we’ve said before… your muscles start working less and less. If you don’t do strength training for a while… they practically waste away by the time you’re over fifty…

Fortunately, it does come back when you start exercising more. But you have to keep maintaining it.

It’s really use it or lose it, and that also applies to the skin cells. So you have to stimulate them to ensure they produce collagen and elastin. And by stimulating them, you get skin tightening.

Constanze: Does that actually have to start from age 25? Because you’re saying it decreases then, but do you have to start then?

Marguérite: Not directly in that sense. It depends a bit on how you age. External factors also contribute to skin aging. We also discussed this in episode 1, about the importance of SPF to protect your skin.

Your skin cells activate as soon as you see signs of aging. Someone with blond hair and blue eyes often ages faster than people with darker skin tones. And as long as you don’t see anything, simply protecting and hydrating are the most important things. But as soon as a problem arises or things bother you, there are other ways to deal with it.

Constanze: And are there any risks or side effects with a non-surgical facelift?

Maxime: Anything that involves injectables, of course, can leave you with a bruise.Filler, yes, if it gets into a blood vessel, you can develop a complication, but that’s essentially a complication we rarely see and rarely occurs.

Constanze: Okay. And how can you achieve such a natural result? Because you still want to be yourself, of course.

Marguérite: That’s also the beauty of a non-surgical facelift: you’re actually treating someone. Because as you get older, there are actually two very important things that happen. One is the skin, which becomes looser. And it’s a kind of bag with contents. And if you look at a balloon that you blow up, when you take it out of the bag, it’s nice and elastic. The moment you blow into it and let it deflate… Is it a flaring issue, or is the elasticity reduced by stretching? That also happens with aging, and volume is also a factor. So all our doctors always look at skin and volume.

Constanze: And you look at each person in their own way…

Marguérite: It’s different for everyone. For example, some people have a very good bone structure, which allows their skin to literally hold their head better, but if you have less cheekbone, for example, it sags a bit more quickly. For others, it’s more the area around the mouth, near the teeth, where the periosteum is more prominent, for example, and then we focus on that. Some people talk all day, and then you also get wrinkles in the skin there. Well, we can treat both the skin and the volume, which is really important to mention, right Maxime?

Maxime: Absolutely.

Constanze: And would it be interesting to briefly mention what treatments you actually have for that Non-Surgical Facelift?

Maxime: Yes, we have different things. We look at different things, as you mentioned. If we look at the skin, at its structure, you can use a Sofwave for firming and tightening, for example.

We have polyfill as an injectable or the Profilo. If you’re talking about volume, you’re more likely to be talking about a filler. If you’re talking about slimming, you’re more likely to be looking at a muscle relaxant.

Marguérite: Sometimes it’s also the outer layer of the skin that has aged due to sun damage, for example. And then we’re happy to refer you to Renske Ypma, Renske Ypma Skin Clinics.

Because they are excellent skin therapists.

Constanze: Interesting. And suppose I want to come to you for a Nonsurgical Facelift. How exactly does it work?

What’s the process and how often do I have to come back?

Maxime: You’ll come to us for a consultation. And then we’ll create a customized plan for you. Because everyone is different. And sometimes it’s a matter of changing the skin. Other times it’s a matter of treating volume. And so you have to create a plan for that.

Anyway… it’s often the case that we don’t do everything at once, so for most treatments we really do take more time in consultations.

Marguérite: Yes, I can always add that, the skin is the organ, so everything we do affects it. So we literally let it happen, because you see that everything you do helps and makes the skin better and better.

But to truly achieve a natural result, that authentic you, just like your face… but then a few years back in time… we also have to take the time for that.

Constanze: And what are the costs, for example, because if I were to have a facelift, I think it would be very expensive.

What about a facelift, a nonsurgical facelift? And how often would I have to come back for that? Marguérite: The price of a really good facelift surgeon… is easily around €40,000/€50,000 in the Netherlands these days. You can also get it cheaper. And they might be very good too. It depends a bit on where you are (in aging). That’s why we have mild, medium, and strong. You can usually start mild from around 40.

Medium really from 50. And strong is for people over 60.

Constanze: Okay.

Marguérite: It’s on our website.

Constanze: Yes, AAYA.nl. Great. I think Susan Hoffman also has questions from viewers. She’ll be in the studio with us shortly. And if you have any questions at home, send us a DM. On Instagram, @AAYAclinics.

AAYA TV — FAQ

The Non-Surgical Facelift — I’m already a fan. And today in the AAYA studio, as always, Susan Hoffman is here, and she’s asking viewers questions about this topic. There are so many people who are very interested. How does it actually work?

Susan?

Susan: Well, here’s the first question: How long does the effect of a Non-Surgical Facelift last?

Maxime: Yes, good question. I think it’s good to know that there are different types of facelifts, especially the surgical ones. So, you have the deep plane facelift. You have superficial ones; the deep plane facelift lasts a bit longer than the more superficial ones. But ultimately, no matter what you do, the skin aging and our own aging just continues.

The beauty of a Non-Surgical Facelift is that we… Non-Surgical Facelifts depend on what the person needs. So perhaps with Polyphil or a Sofwave, in any case, we always assess the individual’s needs until the desired result is achieved. Then, every year, we carefully perform small tweaks, maintaining the condition. This way, it stays beautiful and age-appropriate. And we assess each time what’s needed. So I think the Non-Surgical Facelift will last as long as we continue to perform small tweaks.

Susan: Yes, I’m more of a fan of that too. What’s the recovery like after a non-surgical facelift?

Maxime: Yes, actually, it’s actually quite short.

If you have a surgical facelift… you’re completely out of commission for a week or two. You might not even dare leave the house. But with a non-surgical facelift, I’m talking maybe one or two days. After a Sofwave, you have no downtime at all…

…and you might have a little swelling or a bruise… …that takes a few days to recover from. That’s really the maximum.

Constanze: So it’s actually quite different if you have to incision… …then you’re in for a very long recovery, right?

Maxime: Yes, definitely. Then you’re really out of commission for a few weeks.

I’d rather have an injection then.

Yes, exactly.

Susan: Is everyone suitable for this treatment?

Maxime:

It’s important that people have realistic expectations. I think when you get older, you shouldn’t try to look 18.

Susan: No, absolutely not.

Maxime: But in principle, with realistic expectations, everyone is suitable for your treatment.

Susan: Okay, thank you.

Constanze: Well, I’d say, interesting topic. If you have any more viewer questions, please send us a direct message on Instagram @aayaclinics and we’ll be happy to come back with a good answer next time. And we’ll continue with the Spotlight soon.

AAYA TV — SPOTLIGHT

And we’re talking about how to activate our skin. Also very important for a Non-Surgical Facelift, right Maxime?

Maxime: Very important.

Constanze: Right.

Well, those were some very interesting questions in the FAQ with Susan. And we already said we want to talk about skin activation. Can you tell me what kind of substance or thing I can use to activate my skin?

Marguérite: Yes, because the idea is that skin cells function more slowly as you age, and you want to counteract that aging a bit. We all know you should protect your skin first and foremost, with an SPF every day. There are several ways to activate that skin, but we want to focus on one substance.

Constanze: And that’s called?

Maxime: Retinol.

Constanze: And where can I find it?

Maxime: Where can you find it? In various products. There are tons of products with retinol on the market. Retinol is heavily advertised. In various strengths. You can find lower strengths at the drugstore. But you can also get it with a prescription.

Marguérite: Then you have more vitamin A acid.

Maxime: That is indeed more vitamin A acid.

Constanze: Yes, but I find that interesting. Because, not being a doctor, I don’t know what the right dosage is for me? Should I use a higher or lower dose? Will you tell me? Or how do I know what to buy?

Marguérite: Usually, at the drugstore, as Maxime does indeed say, that’s a lower dosage. Because retinol can also damage or irritate the skin, especially in higher doses. So, manufacturers simply keep it relatively low on the shelves.

And you can buy most good retinols from specialized companies, like skin therapists and cosmetic doctors. The dosages there are somewhat higher, making it more effective.

Constanze: And it’s not a substance you can take in any other way.

Is that really something you apply?

Marguérite: Eating lots of carrots…

Constanze: Okay, carrots.

Marguérite: It’s a derivative of vitamin A, so that’s why you also have the medication from the pharmacy that doctors can prescribe, vitamin A acid, but retinol is a derivative of it that’s less harmful.

And you can get that over the counter.

Constanze: And if you apply that, don’t you have more problems with the sun?

Marguérite: Absolutely.

Constanze: How do you do that?

Marguérite: That’s one of the problems you have to watch out for. And that’s also something; you don’t want to just start using that.

Maxime: No, so you always have to be careful when you treat with that, and especially if you’re using a higher dose, that you don’t expose yourself to the sun too much.

Because your skin can indeed be more sensitive.

Marguérite: No, but yes… You’re wondering if Vitamin A or Retinol is good for your skin.

Then at least go to a doctor or a skin therapist who knows what they’re talking about. And who can give you good advice. Because with too much of the wrong product, you can still damage your skin.

And that’s not the intention. And as Maxime just said, you can just get sunburned because you’re essentially developing baby skin. And you never just put a baby in the sun, no, because that won’t work.

Constanze: Clear. You just mentioned a skin therapist. We happen to have Renske Ypma in our studio later. She’s a skin therapist.

And you guys work together a bit, right? Can you explain? Or not even a bit, quite well, I think.

Marguérite: Maxime is a cosmetic doctor in Zandvoort, among other places. And we’re with Renske Ypma Skin Clinics, where our doctors are.

Maxime: Absolutely.

Marguérite: And Renske works with us on Minervalaan, and she’s right next to Olympiaplein.

And we just really enjoy working together.

Constanze: You said we refer to each other.

Marguérite: Absolutely. And I’ve known Renske for over ten years, and we’ve always had a great connection. And I think she’s an exceptionally good professional.

Constanze: Now I’m really curious. So we’ll see each other in the AAYA Special with Renske Ypma.

Marguérite: Yes.

AAYA TV — SPECIAL

Constanze: Well, we now have Renske Ypma from Renske Ypma Skin Clinics in our AAYA TV studio. Welcome, nice to have you here. And it’s about an interesting topic: the Non-Surgical Facelift. And now we’re going to talk about skin therapy versus cosmetic medicine. Sometimes we really get lost; you can’t see the forest for the trees. When should you choose skin therapy and when should you choose cosmetic medicine?

Renske: Yes, it’s often a combination. So if you’re talking about skin aging, or you want to do something about sagging skin, skin rejuvenation, we often advise our clients to both see a cosmetic doctor and our skin therapists for treatments like removing pigment spots, removing redness, and so on. But also the structure, so more specifically the epidermis — the superficial fine lines, wrinkles, enlarged pores — and the removal of raised areas, so bumps and lumps that can also be present on the skin. We, as skin therapists, also treat that.

Constanze: So, better collaboration for better results.

Renske: Yes, for optimal results. So the injectable treatments that you as cosmetic doctors do… we, as skin therapists, don’t do that. We’re not allowed to do that either. It’s a good thing, after all, shoemaker sticks to his last. Well, it’s good that we all have our specializations. But if you often only opt for injectables… and people still have a lot of pigment spots or redness… or uneven skin, or still that fine or cracked skin, or a dull complexion. That’s really great, that we as skin therapists can treat that. Because then you’re addressing all the layers of the skin.

And yes, if someone still has a lot of sun damage, for example… then their skin still looks a bit old. Also, yes, has someone had injectable treatment? So it’s really a combination.

Constanze: And do you perhaps have a specific example of someone… who has done both, a case…

Renske: Yes, a lot. We actually see that a lot.

So many women and men come to us and point in the mirror, “I’m bothered by these and those wrinkles,” or indeed, the sagging skin, all of which can be treated beautifully with equipment and injectables. We always make a combination appointment. So then they either go to a skin therapist first and then to a cosmetologist, or vice versa.

But one can’t really exist without the other.

Constanze: And what problems do people or clients often face when they’re unsure which professional to go to? Which professional should I go to with which complaint?

Renske: Yes, it’s perfectly logical that most people can’t see the forest for the trees, because there are so many clinics these days; you see something related to beauty on almost every street corner.

There are beauticians and, in the Netherlands, skin therapists. Skin therapy doesn’t exist in other European countries, so it’s a paramedical profession. So, I’m saying we’re working in cosmetology, somewhere between cosmetic and medical. What I find incredibly important is that people go to someone with a lot of experience.

So, if you do something a lot every day, you become good at it. And they also use good equipment. And they should have had proper training.

Constanze: And Maxime, what do you think would be the first step after rejuvenation? If someone wants to do something, what do you recommend? How should I start?

Maxime: Ultimately, I think it’s a bit different for everyone. I think it all starts with a healthy lifestyle for everyone. Because everything you do at home for your lifestyle, everything we do on top of that, makes it more beautiful. But that’s really the foundation. And then you’re talking about your skincare routine, for example, which is of course very important.

Prevention is always better than cure. So using a good SPF is very important, and even if you still have pigment spots, we can still end up with Renske. But I think that’s really the basis for preventing skin rejuvenation.

And any finishing touches we can add to that are welcome.

Constanze: And what other advice do you have for us on how I can keep my skin healthy and prevent aging without cutting into it?

Maxime: It’s kind of the same. It really starts with a healthy lifestyle and protecting your skin.

With early aging skin, so from around 25 or 30, your skin starts to show signs of aging. And then it’s important to activate the skin occasionally. And yes, I think those are the most important things.

Renske: And combining the treatments. And what we sometimes see is that people come to the practice and are like ’I’d like micro-needling,’ for example and then we can give someone six micro-needling treatments but we actually just look at each session also what someone needs so if someone comes in six weeks their skin could look completely different some inflammation and then we don’t do certain treatments.

So we always create a customized treatment plan, just like you do and we combine many treatment healings and our Sofwave treatments with radiofrequency and ultrasound. So, of course, you have to look at each individual, but also at each treatment, yes.

Constanze: Well, interesting. I’d say, give it a try. Also take a look at the Renske Ypma website, Renske Ypma Skin Clinics.

And keep watching! If you have any questions, send them via DM: @aayaclinics on Instagram.

AAYA TV — HOLISTIC HACK

Red Light Therapy — we’ve heard a lot about it, but there are so many different devices and treatments. What should I do?

Marguérite: You often see people wearing masks like that. They appear on Instagram or something.

Constance: Yes. Is that it?

Maxime: That could be it.

Marguérite: Yes, and you can certainly buy such a good device. But I think the problem then lies in doing it often enough.

Constanze: So what does Red Light Therapy actually do?

Maxime: That red light, that red light, actually stimulates your cells. Or the little factories in your cells to become more active. It’s actually a bit like activating collagen production.

Constanze: So this is my very own Non-Surgical Facelift for home use.

Marguérite: Yes, but you have to do it very often.

Constanze: How often?

Maxime: At least several times a week. And if you want short-term results, then as often as possible. So you could do it every day, so to speak. And if you want long-term results, then it would have to be at least twice a week.

Constanze: Can he recommend anything someone should buy? Or ask you questions? Or send me a DM?

Maxime: That’s always welcome. And I would always send a message back. We’re always happy to answer questions. But the most important thing is to ensure the correct wavelength of light is present.

Marguérite: The Holistic Hack is Red Light Therapy. Yes, it works. Only adherence to the therapy determines its success.

Constanze: So I’d say, give it a try yourself (or not)

And then send us a message to let us know how it went, or not.

Marguérite: It seems a bit tricky to me. I don’t have time, but that’s the problem.

Constanze: Still very interesting, though. We’ll continue with our wrap-up soon. And then we’ll see each other again soon.

AAYA TV — WRAP-UP

Very interesting, this episode about the Non-Surgical Facelift with super guests like Renske Ypma. And then of course we had Susan, Susan Hoffman, Maxime, and Marguérite. What do you think is the most important lesson we learned today?

Marguérite: Actually, there are two.

Maxime: I think there’s definitely one…

Marguérite: Or did you have three?

Maxime: Well, let’s start with one. I think there’s definitely a good alternative to a surgical facelift. A non-surgical facelift. Where a combination of cosmetic medicine and skin therapy produces the most beautiful results.

Marguérite: That was actually my second.

Maxime: Oh, that was your second?

Marguérite: Yes.

So, on the one hand, we don’t have to incise. I really like that. We have an alternative. And the collaboration with the skin therapists.

Constanze: That’s good too, isn’t it?

Marguérite: As consumer doctors. Well, then I want to take two more classes here.

Constanze: Do you have any questions at home? Just send us a DM @aayaclinics.

Then we can address them next time. Or you’ll just get an answer. Don’t forget to subscribe to AAYA TV on YouTube. Or listen to our podcast on one of your favorite podcast platforms. And I hope to see you again in the next episode. And don’t forget one more thing: Stay amazing as you are!

Maxime: Renske Ypma.

Marguérite: Renske Ypma. Zij is huidtherapeut van Renske Ypma Skin Clinics.

Constanze: En jullie werken ook samen toch in Zandvoort?

Marguérite: Wij werken samen in Zandvoort, Amsterdam…

Constanze: Leuk.

AAYA TV — DOCTORS CONSULT

Maxime: Dat is goed.

Want iedereen is anders.

Constanze: Ja, AAYA.nl. Fijn.

Maxime: Heel erg belangrijk.

Constanze: Juist.

Maxime: Op Retinol.

Constanze: En waarin vind ik dat?

Maxime: Waarin vind je dat? In verschillende producten. In verschillende sterktes.

Zij is huidtherapeut.

…micro-needling behandelingen toepassen…

Constanze: Hoe vaak?

Maxime: In ieder geval meerdere keren per week.

Maxime: Dat mag altijd.

Constanze: Toch alsnog heel interessant.

AAYA TV — WRAP-UP

Marguérite: Dat was eigenlijk mijn tweede.

Of jullie krijgen gewoon antwoord. En nog één ding niet vergeten Blijf amazing as you are!
