Cold plunge has moved from elite sports facility into mainstream physical therapy practice. The equipment got better and cheaper, patients started asking for it, and the cash-pay revenue model works for clinics that don’t want to fight insurance for every billable code.
A few things worth knowing before you buy.
It’s not the same equipment as a home plunge
A clinic plunge sees multiple patients a day — including post-surgical, immunocompromised, and elderly patients. The requirements that follow:
- 1 HP commercial chiller minimum (1.5 HP for high volume)
- Built-in ozone sanitation — manual chemical treatment between patients isn’t realistic in a clinical setting
- Adjustable temperature across 0–15°C for clinical flexibility
- Quiet operation — treatment rooms are next door
- Documentation logs for clinical records and infection control
Consumer-grade units don’t have these features and won’t survive clinical use. Specify commercial from the start.
The business case isn’t insurance
Insurance doesn’t reimburse cold plunge as a standalone code. The clinics making money from it use cash-pay models:
- Recovery sessions at $25–50
- Add-on to PT visits at $15–25

Protocols matter as much as equipment
Contraindications screening, temperature selection, session duration, supervision for first-time users, documentation — all of this needs to be in place before you start treating patients. Sourcing
Domestic distributors mark up 30–50% over factory pricing. For a single clinic, factory-direct still makes sense if you can accommodate 4–8 weeks of lead time.
