Tis the season. Flu, COVID-19, norovirus, RSV, and a dozen other viruses are making the rounds. Suddenly everyone you know is sick, was just sick, or is about to be. Your kid’s school sends another exposure notice. Your coworker is “powering through” with a suspicious cough. The person next to you on the train just sneezed into the void.
Your hands are in the middle of it all. They touch everything — door handles, shopping carts, your phone, your face (yes, more than you think). That makes hand hygiene one of the simplest and most effective ways to protect yourself and others.
Soap and Water: The Gold Standard
The gold standard is soap and water. Full stop. But life happens. Maybe your kid needed to pee in the middle of nowhere, and the gas station hasn’t seen a soap refill in years. That’s when hand sanitizer comes in.
Soap physically removes germs from your hands. Sanitizers work differently: the alcohol attacks the structure of certain pathogens, dissolving the fatty outer layer of enveloped viruses like flu and COVID-19. Both work, but not interchangeably, and not against everything. Nonenveloped viruses like norovirus are tougher and need physical removal via handwashing.
Understanding What’s on Your Hands
Bacteria
Bacteria are single-celled organisms without a nucleus. Gram staining classifies them based on cell wall structure:
Gram-positive bacteria have thick walls made of peptidoglycan and include Staphylococcus aureus and Streptococcus. They often produce exotoxins that can harm tissues.
Gram-negative bacteria have thinner walls but an additional outer membrane rich in lipopolysaccharides. E. coli and Pseudomonas aeruginosa fall into this group. The outer membrane can make them more resistant to disinfectants, which is why scrubbing is crucial.
Viruses
Viruses aren’t cells. They’re genetic material wrapped in a protein coat, sometimes with a lipid envelope.
Enveloped viruses, like flu, SARS-CoV-2, and RSV, have a fragile lipid layer that sanitizers dissolve. Nonenveloped viruses, like norovirus, lack this layer, making them resistant to alcohol. Soap and scrubbing physically remove them.
How Soap Works
Soap contains surfactants — molecules with a water-loving head and oil-loving tail. This allows soap to grab onto oily particles, including germs, and suspend them in water so they can be rinsed away.
Bar soap and liquid soap are equally effective. Modern bars are often syndet bars with gentler surfactants, while liquid soaps use similar chemistry. The “antibacterial” label is mostly marketing; regular soap works just as well for everyday handwashing.
Proper Handwashing Technique
Wet your hands with clean, running water. Apply soap and lather, covering backs of hands, between fingers, and under nails. Scrub for at least 20 seconds. Rinse thoroughly under running water. Dry with a clean towel or air dry.
Focus on thumbs, fingertips, and nail spaces. Water temperature doesn’t matter — cold water works fine, and hot water can dry your skin.
Hand Sanitizers: When Soap Isn’t Available
Alcohol-based hand sanitizers (60–95% ethanol) are excellent when soap and water aren’t available. They work by denaturing proteins and disrupting lipid membranes, effective against enveloped viruses.
Sanitizers are OTC drugs regulated by the FDA. Products must pass standardized tests to ensure antimicrobial effectiveness. Too much alcohol (100%) isn’t ideal; water is needed for proper protein denaturation.
Gels are thickened to prevent dripping and slow evaporation, increasing contact time. Humectants like glycerin keep skin from drying out.
Rinse-Free Soaps and Other Alternatives
Rinse-free or waterless soaps physically lift dirt and microbes from the skin. They can be useful outdoors or in emergencies but don’t kill germs like alcohol-based sanitizers. They are an option, not a replacement for soap and water.
