🗝️ Key Learning Points

Definition

  • AAA = Permanent dilation of the abdominal aorta ≥3 cm in diameter, most commonly below the renal arteries.
  • Mnemonic: “Aneurysm = Artery Always Enlarged” (≥3 cm).

Causes & Risk Factors (Mnemonic: AGETHATS)

  • Age >65
  • Gender (Male)
  • Every puff counts (Smoking)
  • Tension (Hypertension)
  • History (Family history)
  • Artery hardening (Atherosclerosis)
  • Tissue disorders (e.g., Marfan syndrome)
  • Situations (Inflammation, infection)

Pathophysiology

  • Weakening of aortic wall due to breakdown of elastic fibres, chronic inflammation, and atherosclerosis.
  • Gradual bulging under pressure — risk increases as diameter increases.

Symptoms

  • Silent killer: Most AAAs are asymptomatic until rupture.
  • Possible vague abdominal/back/loin pain or a palpable pulsatile mass.
  • Rupture = Sudden severe pain, collapse, shock (Classic triad).

Differential Diagnosis

  • Renal colic, musculoskeletal back pain, GI causes (diverticulitis, gallstones), aortic dissection, iliac aneurysm.

Investigations

  • Screening & diagnosis: Abdominal ultrasound (quick, non-invasive, gold standard for screening).
  • Pre-op/rupture: CT angiography for surgical planning or suspected rupture.
  • Bloods: FBC, U&Es, LFTs, clotting, group & save, ECG, +/- CXR.

Management

  • Surveillance for small AAAs:
  • 3–4.4 cm: yearly US.
  • 4.5–5.4 cm: 3-monthly US.
  • Elective repair if:
  • Diameter ≥5.5 cm, rapid growth (>1 cm/year), or symptomatic.
  • Repair options:
  • Open surgical repair (synthetic graft).
  • EVAR (Endovascular Aneurysm Repair) — less invasive, stent graft via femoral arteries.
  • Medical management: Smoking cessation, blood pressure control, statins, antiplatelets, manage comorbidities.
  • Rupture = Emergency surgery, resuscitation.

Complications

  • Rupture (massive internal bleeding, ~80% mortality)
  • Aortic dissection
  • Peripheral embolisation (limb ischaemia)
  • Graft complications (endoleak, infection)
  • Compression of nearby structures (ureters)

Prognosis

  • Excellent if small and managed proactively.
  • Large, untreated = high rupture and mortality risk.
  • After elective repair: ~70% 5-year survival.
  • Ruptured AAA: <50% survive to hospital discharge even with surgery.
