As a Physician Assistant, I spend a lot of time reviewing lab results, blood pressure readings, imaging studies, and medication lists.
Yet one of the simplest measurements we can obtain often receives far less attention than it deserves:
Resting heart rate (RHR).
Most people know their weight.
Many know their cholesterol.
Some know their blood pressure.
But very few know their resting heart rate — and what it may reveal about their overall health.
The truth is that your resting heart rate is more than just a number on a smartwatch. It reflects how efficiently your heart works, how well your nervous system is functioning, your level of physical fitness, and potentially even your long-term risk of disease and premature death.
What Is Resting Heart Rate?
Resting heart rate is the number of times your heart beats per minute while you are awake, relaxed, and at rest.
For most adults, a normal resting heart rate ranges between 60 and 100 beats per minute.
However, “normal” and “optimal” are not always the same thing.
Many healthy adults have resting heart rates significantly below 60 beats per minute, especially those who exercise regularly.
What Is Considered an Ideal Resting Heart Rate?
Research consistently shows that lower resting heart rates within a healthy range are associated with better cardiovascular outcomes.
A practical guide:
- 80+ bpm: Higher than ideal
- 70–79 bpm: Average
- 60–69 bpm: Good
- 50–59 bpm: Excellent
- Below 50 bpm: Common in highly trained individuals
This does not mean everyone should aim for a resting heart rate of 45.
The goal is not to chase the lowest possible number.
The goal is to improve cardiovascular fitness and overall health.
A low resting heart rate is often a result of those improvements — not the objective itself.
Why Does Resting Heart Rate Matter?
Every heartbeat requires energy.
When the heart becomes stronger and more efficient, it can pump more blood with each contraction.
This means it does not need to beat as frequently to deliver oxygen and nutrients throughout the body.
Think of it this way:
A stronger heart accomplishes more work with fewer beats.
Over decades, that efficiency matters.
The Longevity Connection
One of the most interesting findings in cardiovascular research is the relationship between resting heart rate and mortality.
A large meta-analysis published in CMAJ found that higher resting heart rates were associated with increased risks of cardiovascular disease and all-cause mortality.
Individuals with resting heart rates above 80 beats per minute had significantly higher risks compared with those whose resting heart rates were lower.
Importantly, this relationship was observed even among people without known heart disease.
In other words, resting heart rate appears to be more than just a marker of fitness — it may also serve as a marker of overall physiological stress.
What Happens as We Age?
Aging itself does not necessarily cause resting heart rate to increase dramatically.
However, several factors commonly associated with aging can influence it:
- Reduced physical activity
- Loss of aerobic fitness
- Weight gain
- Poor sleep
- Chronic inflammation
- Certain medications
- Cardiovascular disease
The encouraging news is that resting heart rate remains highly modifiable throughout life.
Studies show that adults can improve cardiovascular fitness and lower resting heart rate through regular aerobic exercise even in their 60s, 70s, and beyond.
The Role of the Nervous System
Your resting heart rate reflects a balance between two branches of the autonomic nervous system:
Sympathetic Nervous System
Often called the “fight-or-flight” system.
This increases heart rate and prepares the body for action.
Parasympathetic Nervous System
Often called the “rest-and-digest” system.
This slows heart rate and promotes recovery.
Individuals with greater parasympathetic activity generally have lower resting heart rates and better cardiovascular fitness.
This is one reason exercise, meditation, adequate sleep, and stress management can positively influence heart rate.
What Can Raise Resting Heart Rate?
Several factors can temporarily or chronically elevate resting heart rate:
- Poor sleep
- Psychological stress
- Illness
- Fever
- Dehydration
- Excess alcohol
- Smoking
- Excess caffeine
- Certain medications
- Overtraining
- Obesity
- Untreated sleep apnea
A sudden increase of 5–10 beats per minute above your usual baseline may sometimes be an early signal that your body is under stress.
What About Smartwatches?
Modern wearable devices have made resting heart rate tracking easier than ever.
While no wearable is perfect, devices such as the Apple Watch, Oura Ring, Garmin, and Fitbit generally provide useful trend data.
The trend is often more important than any single reading.
Ask yourself:
- Is my resting heart rate stable?
- Is it gradually improving?
- Has it suddenly increased?
Changes over time are frequently more meaningful than one isolated measurement.
How Can You Improve Your Resting Heart Rate?
The most effective strategies are also some of the simplest:
Regular Aerobic Exercise
Walking, jogging, cycling, swimming, and other cardiovascular activities strengthen the heart.
Maintain a Healthy Weight
Excess body weight increases cardiovascular workload.
Prioritize Sleep
Poor sleep is strongly associated with elevated resting heart rate and increased cardiovascular risk.
Manage Stress
Meditation, mindfulness, social connection, and recovery practices can support autonomic balance.
Avoid Tobacco
Smoking consistently raises heart rate and cardiovascular risk.
Limit Excess Alcohol
Alcohol can impair sleep and increase resting heart rate.
The Bottom Line
Your resting heart rate is one of the simplest measurements available — and one of the most informative.
It reflects cardiovascular fitness, autonomic nervous system function, recovery status, and overall health.
While there is no single “perfect” number for everyone, lower resting heart rates within a healthy range are generally associated with better cardiovascular outcomes and longevity.
You do not need elite-athlete numbers.
You simply need a heart that is becoming more efficient over time.
Sometimes the most meaningful health markers are not found in expensive tests or advanced imaging.
Sometimes they are already sitting on your wrist.
