Yet his BMI score places him at the upper boundary of normal, bordering on overweight. The formula behind that score divides the weight by the square of the height. It does not ask how much of that weight is muscle. It does not ask how much fat there is. It simply divides and classifies.

This is not a rare contradiction. BMI was developed in the 1970s by Ancel Keys as a population-level screening tool for large epidemiological studies. It was never intended as an individual diagnostic. Yet clinical settings worldwide adopted it as a primary health indicator because it was fast, free, and required nothing more than a scale and a tape measure. That convenience came with a significant blind spot: the formula treats every kilogram the same, whether it is dense muscle tissue or visceral fat surrounding the organs.

Body fat percentage corrects for this by measuring the actual proportion of fat to lean mass. The American College of Sports Medicine defines essential fat at 10% for women and 3% for men, with athletic ranges of 6–13% for men and 14–20% for women. These thresholds vary further by sport. Male bodybuilders target 5–8%. Female basketball players maintain a healthy body fat between 20 and 27%. A single BMI cut-off cannot account for any of this variation.

Measurement accuracy adds another layer. Consumer Reports tested six BIA body fat scales against the Bod Pod, a laboratory-standard device. The most accurate scale still deviated by 21%. The least accurate was off by 34%. Skinfold callipers carry error rates up to 8% and depend on practitioner skill. DEXA scans and hydrostatic weighing deliver the highest precision but require specialist equipment and incur greater costs.

The strongest health assessment does not rely on any single metric. Combining body fat percentage with waist circumference, waist-to-hip ratio, and a practical self-assessment provides a picture that one number alone cannot. Visceral fat, the deep fat surrounding organs, is metabolically active and directly linked to insulin resistance, fatty liver disease, and cardiovascular risk. Waist measurements under 36 inches for men and 35 inches for women are associated with lower risk profiles.

Health is not a score. It is a composite of signals that must be read together, personalised to the individual, and interpreted with the full context of how that body moves, performs, and functions.
