Beyond the 20/20: What a Comprehensive Eye Exam Reveals About Your Overall Health

Our eyes can reveal a lot about our overall health, and I learned that lesson the hard way. A few years ago, my daughter started showing symptoms that I, being an optometrist, simply mistook for an eye infection. She had redness, excessive watering, light sensitivity, and even said her vision felt blurry. I took her to an ophthalmologist, and what he revealed was shocking. It wasn’t an infection at all, but a consequence of an underlying systemic disease. We got her tested, and she was soon diagnosed with juvenile idiopathic arthritis.

What Your Eyes Are Telling Us

After my daughter’s incident, I decided not to overlook any symptoms our eyes show. Now, being an optometrist, I feel it’s important to educate people on basic eye conditions that could be related to any systemic disease. This would, to some extent, help people diagnose their systemic illness earlier. Here are a few of them.

Diabetes:

Often, a simple routine eye exam can act as the first clue that a patient has diabetes – even before they experience any other symptoms. When we look at the retina, we can see tiny changes in the blood vessels, such as small leaks, bleeding spots, or fragile new blood vessels. These visible signs prompt an optometrist to inquire about other symptoms, like unusual thirst or frequent urination, leading to an early diagnosis and treatment.

Thyroid:

A doctor may suspect thyroid disease during an eye exam if they see bulging eyes, lid changes, pain during eye movement, or corneal exposure. These signs lead to further thyroid testing for confirmation. Thyroid eye disease, particularly in Graves’ disease, can cause significant ocular complications if left unaddressed – making early detection during a routine exam genuinely valuable.

Hypertension:

Changes in the retina’s blood vessels – narrowing, leakage, or bleeding – act as a mirror of what high blood pressure is doing to blood vessels throughout the body. If these signs are seen in a patient without a hypertension diagnosis, the eye doctor recommends they get their blood pressure checked. The patient is then referred to a physician for confirmation and treatment.

Cholesterol:

An eye exam can reveal high cholesterol through rings in the cornea, fatty eyelid deposits, or cholesterol plaques in retinal vessels – all of which are early warnings of cardiovascular risk.

Autoimmune Diseases:

An eye exam can reveal an autoimmune disorder through inflammation in the eye (uveitis, scleritis, vasculitis, optic neuritis), which may be the first sign of diseases like rheumatoid arthritis, lupus, or multiple sclerosis.

HIV Infection:

An eye exam can reveal HIV infection because HIV weakens the immune system, allowing unusual infections and vascular changes to appear in the eye. Sometimes the first signs of HIV/AIDS in a retinal examination are commonly retinal hemorrhages, unusual infections, or eyelid tumors.

Subtle changes in the retina or eye tissues often point to conditions like diabetes, hypertension, thyroid disorders, or autoimmune diseases – sometimes before patients even know they have them.
