The 4x4 Protocol That Outperforms Standard Cardio by Every Measure
VO2 max is the maximum volume of oxygen the body can utilise during intense exercise. It reflects how efficiently the heart, lungs, and muscles work together under demand. It is directly correlated with reduced cardiovascular disease risk and lower all-cause mortality. Among fitness metrics, it is the closest thing to a longevity predictor.
The HIIT 4x4 protocol was developed at the Norwegian University of Science and Technology in Trondheim specifically to improve it. The structure is precise: four minutes at 85–95% of peak heart rate, corresponding to 90–95% of maximal oxygen uptake, followed by two to three minutes of active recovery at 60–70% of peak heart rate. Four rounds. A warm-up before and a cool-down after frame the session.
The intensity is deliberate. Research has established that improvements in VO2 max and stroke volume are intensity-dependent. The most substantial gains occur in those who train at high intensity rather than moderate continuous effort. The 4x4 protocol systematically exploits this threshold.

The data support the design. VO2 max improvements of 16.5% were recorded against moderate-intensity continuous training. In coronary artery disease patients, gains reached 17.9%, double the conventional approach. Some studies documented improvements of up to 35%. Beyond aerobic capacity, the protocol reversed signs of cardiac ageing, enhanced arterial elasticity, and reduced vascular stiffness, particularly in older adults.
The individual cases are equally striking. A 42-year-old with a spinal cord injury achieved a 52% increase in peak aerobic power after 12 weeks. A 70-year-old woman improved her cardiorespiratory fitness by 36%, shifting her surgical risk classification from high-risk to operable. Older adults with hypertension saw significant blood pressure reductions without medication adjustments.
The protocol scales from novice to elite. Beginners start with shorter intervals at manageable intensity and progress as capacity builds. Experienced athletes implement supramaximal efforts and varied interval configurations. Heart rate monitoring provides objective data to precisely calibrate each session. Integration requires three steps: establish a personal baseline, apply progressive overload, and monitor consistently.
What makes this protocol compelling is not just the numbers. It is the combination of accessibility, scalability, and a depth of clinical evidence that most training methods simply do not have.
