The story begins in late eighteenth- and early nineteenth-century Württemberg, Germany, where the slaughter of swine was both necessity and ritual. Little was wasted. Blood, fat, and organ meats were gathered and worked into large sausages — Blutwurst, dense, dark cylinders packed into thick intestinal casings, natural sleeves drawn from the animal itself — then preserved by smoking or air-drying.
Prepared during the cold months, they were kept in unheated attics, cellars, or smokehouses, where the ambient chill served as a natural preservative. Frost slowed decay; smoke deepened both flavor and durability. In this way, the yield of a single slaughter could be carried forward for weeks or months, sustaining a household into the early spring.
By the late eighteenth century, a recurrent and often fatal illness in the rural districts of Württemberg had come to be known as “sausage poisoning” (Wurstvergiftung). In farming households, entire families or groups of neighbors could fall ill after sharing the same food. These were not isolated accidents but repeated calamities, marked by a distinctive syndrome and a striking association with particular sausages. One of the best-known outbreaks occurred in 1793 at Wildbad, where thirteen people became ill after eating blood sausage and six died. The case later assumed historical importance as an early, well-documented episode that would eventually engage the attention of a local doctor.
Justinus Kerner (1786–1862) — a practicing physician who entered Württemberg’s public medical service and served as a district medical officer — was one of the more intriguing figures in nineteenth-century German cultural life. In addition to his medical career, he was a Romantic poet and polymath who moved within the Swabian circle of writers and intellectuals in southwestern Germany. His interests extended beyond medicine and literature into stranger territory. He became widely known for his investigations into somnambulism and the paranormal, most notably through his account of the Seeress of Prevorst, a work often seen as an early exploration of phenomena later discussed in psychiatric or dissociative terms.
It was not from the salons of poetry or the darkened rooms of somnambulists that Kerner’s most enduring work emerged, but from his investigation of the mysterious and often fatal malady known as sausage poisoning. He reviewed case after case from across the region, turning scattered rural tragedies into a recognizable clinical pattern. By 1820 he had assembled 76 patient histories in his first monograph; by 1822, that number had grown to 155 cases. His descriptions of the poisonings were remarkable for their careful detail. He also conducted experiments, testing its effects on himself and on animals.
From this analysis he recognized a striking and consistent clinical pattern: after a delay of hours to days following sausage consumption, patients developed dryness of the mouth and throat, difficulty swallowing, and visual disturbances, followed by a steadily advancing muscle weakness that could culminate in respiratory failure. Notably absent were fever and delirium. What impressed him most — and set the illness apart from the fevers and toxic states of his time — was the preservation of mental clarity: patients remained fully conscious and aware even as paralysis progressed.
Without any knowledge of bacteria, Kerner effectively characterized the illness of sausage poisoning, correctly attributing it to a toxic substance formed during sausage preparation and storage — now known to be produced by Clostridium botulinum. It was not until the late nineteenth century that the condition was formally named botulism, from the Latin botulus, meaning “sausage.”
What elevates Kerner’s work beyond clinical description is the reach of his imagination. From the narrow circumstances of rural food poisoning, he discerned not only the nature of a lethal toxin but its latent possibilities. He speculated that a substance capable of silencing overactive nerves might, in controlled form, be harnessed for therapeutic ends. It was an extraordinary intuition: that the very agent responsible for paralysis could, in measured doses, relieve the pathological excess of muscular contraction. In this, Kerner was more than an early toxicologist — he anticipated the therapeutic use of botulinum toxin.
We now understand that botulinum toxin acts by blocking the release of acetylcholine, the neurotransmitter that signals muscles to contract. In doing so, it interrupts nerve transmission and produces a temporary relaxation of muscle.
The clinical use of BOTOX® (onabotulinumtoxinA) has developed over more than 45 years. First studied in humans in the 1970s, it was initially used to treat eye muscle disorders, with ophthalmologist Alan Scott performing the first injection for strabismus in 1978. The FDA approved BOTOX in 1989 for strabismus and blepharospasm. Cosmetic use came later, with approval in 2002 for the temporary improvement of glabellar frown lines. Today, onabotulinumtoxinA is widely used not only for aesthetic purposes but also for chronic migraine, severe axillary hyperhidrosis, overactive bladder, spasticity, and a range of neurologic and muscular disorders.
Its therapeutic reach has continued to widen, and investigators are exploring additional applications. Studies have examined whether botulinum toxin may have a role in disorders such as depression, while intradermal techniques are being studied in dermatology for skin texture and conditions such as rosacea. Other areas under investigation include tremor and selected gastrointestinal disorders, though many such uses remain experimental. Meanwhile, longer-acting botulinum toxin formulations are beginning to enter clinical practice, while others remain in development, suggesting that this family of agents may continue to broaden its clinical role.
In the end, the story returns to Justinus Kerner. From the dim kitchens and cellar rafters of rural Württemberg, he drew from scattered tragedy a coherent pattern, and from that pattern an idea that would outlive him by centuries. Working without knowledge of microbiology or the tools of modern science, he recognized in sausage poisoning not only a deadly toxin but a principle — that forces capable of harm might, in careful hands, be turned toward healing. It is perhaps no accident that he was also an accomplished and highly regarded German poet, for such insight required not only clinical rigor but a certain imaginative reach. It is a rare kind of vision, at once scientific and lyrical, and it places Kerner among those physicians whose insights do not merely explain disease, but continue to shape the future of medicine.
