Features such as optic disc edema and a markedly raised erythrocyte sedimentation rate (ESR) may be absent, potentially creating a false sense of reassurance.

Subtle neuro-ophthalmic signs may be the key to diagnosis. Ocular cranial nerve paresis, diplopia, and other visual disturbances can sometimes be the earliest manifestations. In some patients, retinal findings such as microaneurysms may reflect metabolic control that is reasonable but not entirely optimal, adding another layer of complexity to the clinical picture.

The Importance of a Broad Differential and Multidisciplinary Care

Because atypical presentations can delay diagnosis, maintaining a high index of suspicion is essential. A thorough history, detailed ocular examination, and appropriate investigations should guide management. When neurological symptoms or cranial nerve involvement are present, a neurology referral can be particularly valuable. Collaboration between ophthalmology, neurology, and primary care teams may help identify the underlying cause sooner and reduce the risk of potentially irreversible visual complications.
