Why “Operator Error” is NOT a Root Cause in GMP
One of the most common issues in GMP deviation reports is identifying the root cause as “operator error.”
At first glance, it seems reasonable.
An operator made a mistake. So the cause is human error.
But from a regulatory perspective, this is not considered an acceptable root cause.
Why “operator error” is not enough
Regulators expect you to identify why the operator made the mistake.
For example:
- Was the SOP unclear?
- Was the training inadequate?
- Was the process too complex?
- Were there environmental or time pressures?
If you stop at “operator error,” you are not addressing the real problem.
What auditors look for
Auditors are not looking for blame.
They are looking for:
- System weaknesses
- Process gaps
- Control failures
If your root cause does not identify a system-level issue, your CAPA will likely be weak.
Weak vs Strong Root Cause
❌ Weak: “The deviation was caused by operator error.”
✔ Strong: “The SOP did not clearly define the sequence of actions, leading to inconsistent execution by operators.”
How to improve your deviation analysis
Instead of stopping at human error, ask:
- Why was the error possible?
- Why was it not detected earlier?
- What controls were missing?
A structured approach
To make this process more consistent, I use a simple risk-based approach to:
- Identify true root causes
- Structure CAPA
- Strengthen audit justification
