Severe bone loss does not always have a single treatment solution.

At Point Dental Clinic, every complex implant case begins with a detailed clinical examination and CBCT analysis. Our goal is not simply to place implants, but to select the safest and most predictable treatment based on each patient’s unique anatomy.

The Patient

A 56-year-old patient from Romania visited Point Dental Clinic after years of wearing a removable upper denture. Her goal was clear: to replace the removable prosthesis with fixed teeth supported by dental implants.

Several clinics in Romania, Hungary and Switzerland evaluated her case. Because of the severe bone loss in the posterior right side of the upper jaw, they concluded that implant treatment was not possible and no alternative solution was offered.

The Initial Treatment Plan

After evaluating the CBCT scan, the left side of the upper jaw was suitable for three conventional dental implants.

The right side, however, presented a much greater challenge.

The remaining bone volume was insufficient for conventional implants, so a zygomatic implant was initially considered.

The Decision Changed

A more detailed evaluation of the patient’s cheekbone anatomy demonstrated that a zygomatic implant would not provide the safest and most predictable outcome.

Rather than forcing a standard solution, our surgical team selected a different approach.

A patient-specific subperiosteal implant was digitally designed using a dedicated CBCT scan and CAD/CAM planning.

Following approximately 40 days of manufacturing, the patient returned to Point Dental Clinic for surgery.

During the same treatment, the custom-made titanium subperiosteal implant was fixed to the remaining maxillary bone, while the left side had already been restored with three conventional implants during the first visit.

Why This Case Matters

Advanced implantology is not about performing the same procedure for every patient.

It is about understanding anatomy and selecting the treatment that offers the greatest long-term predictability.

Sometimes that means choosing a zygomatic implant.

Sometimes it means choosing a completely different solution.

Current Status

The patient is currently in the healing phase while osseointegration progresses.

She will return to Point Dental Clinic in approximately five months for the delivery of her titanium bar-supported fixed prosthesis.

This case demonstrates how personalized treatment planning can create solutions for patients who have previously been told that implant treatment was not possible.
