How To Properly Use An Epi Pen For Allergic Reactions

An EpiPen® (epinephrine auto-injector) is a life-saving tool used to treat severe allergic reactions (anaphylaxis). Anaphylaxis can happen quickly and become dangerous within minutes, so knowing exactly how to use an EpiPen the right way can make a huge difference in an emergency. Whether the reaction is caused by foods like peanuts or shellfish, insect stings, medications, or unknown triggers, epinephrine is the first and most important treatment.

When should you use an EpiPen?

An EpiPen should be used immediately if someone is showing signs of a severe allergic reaction. Common symptoms include trouble breathing, wheezing, swelling of the lips, tongue, or throat, tightness in the chest, widespread hives, severe itching, dizziness, fainting, or a sudden drop in blood pressure. Some people also experience nausea, vomiting, or stomach cramps during anaphylaxis. If a person has a known severe allergy and is exposed to their allergen, it’s better to give epinephrine early rather than waiting to see if symptoms worsen.

Step-by-step: How to properly use an EpiPen

Before using the device, stay calm and act quickly. Have the person sit or lie down, especially if they feel faint. If they are vomiting or having difficulty breathing, position them on their side to help keep their airway clear.

1. Remove the EpiPen from its carrier tube. Hold it in your fist with the orange tip pointing downward. Never put your thumb over either end of the device.
2. Remove the blue safety cap. This unlocks the injector. Once the cap is off, the device is ready to use.
3. Inject into the outer thigh. Place the orange tip against the middle of the outer thigh. It can be injected through clothing if needed, including jeans.
4. Push firmly until you hear a click. The click means the injection has started.
5. Hold it in place. Keep the EpiPen pressed firmly against the thigh for several seconds to ensure the full dose is delivered.
6. Remove the injector and massage the area. After removing it, gently rub the injection site for about 10 seconds.

What to do immediately after giving epinephrine

Call 911 right away, even if the person starts to feel better. Anaphylaxis can return, and the person needs medical evaluation and monitoring. Emergency responders may provide additional treatment like oxygen, breathing support, or more epinephrine if symptoms come back.

If the person has trouble breathing, keep them as still and comfortable as possible. If they stop breathing or lose their pulse, begin CPR immediately and continue until help arrives.

What if symptoms come back?
