There is a threat to your eyesight that does not come from your genes. It does not come from your diet. And it does not come from your blood sugar alone. It comes from the air you inhale every single day.
A large study published in 2025 tracked over 114,000 adults and found something that deserves attention. Exposure to high levels of particulate matter in the air was directly associated with an increased risk of diabetic retinopathy. The same study also linked it to a higher risk of retinal detachment. The researchers concluded that air pollutants should be considered when discussing risk factors for vision‑threatening eye diseases.

A separate 2025 study examined occupational exposure to air pollutants across over three thousand counties. It found a significant association between exposure to ammonia, sulfur dioxide, and volatile organic compounds and higher rates of diabetic retinopathy.
The evidence is converging. Multiple studies, across multiple populations, all point in the same direction. The air we breathe affects more than our lungs. It reaches the smallest blood vessels in the body, including those that nourish the retina.
The Screening Gap
The American Academy of Ophthalmology recommends that every person with type 2 diabetes undergo a comprehensive dilated eye examination at the time of diagnosis. And at least yearly thereafter.
Yet globally, only about 60 percent of people with diabetes receive these annual eye exams.
This gap means millions of people are living with retinal damage they do not know about. All while being exposed to environmental factors that may be making the condition worse.
In many regions, the screening rates are even lower. The 2025 Revised National Guidelines for the Management of Diabetic Retinopathy highlight that a large proportion of diabetic patients have never undergone a retinal examination. This is a missed opportunity. Because early detection can stop the progression before vision is lost.
What You Can Do?
You cannot control the air quality of an entire region. But you can protect your eyes from the damage that air pollution may accelerate.
First, get a dilated eye examination every year. If you have diabetes, this is the single most important action you can take. The AAO guidelines are clear on this. Catching diabetic retinopathy early, before the leaking vessels cause irreversible macular swelling or new fragile vessels begin to bleed, is the only way to prevent permanent vision loss. Second, reduce your personal exposure to polluted air.
Wear a protective mask when outdoor pollution levels are high. Use an air purifier indoors if possible. Avoid outdoor exercise during peak pollution hours. Keep windows closed on days when the air is visibly hazy or when air quality alerts are issued.
These measures will not reverse existing retinal damage. But they can reduce the inflammatory burden that polluted air places on your blood vessels each day. And for the vessels inside your eyes, that reduction matters.
